What is an end of life planner?

Let’s start with the basics. What is it? 

An end of life planner is a binder that keeps all your crucial information and final wishes in one place. The perfect product for my fellow organizational queens and control freaks.

Why should I have an end of life planner?

There are a plethora (yes, I’ve been using this word in every article – It’s my new favorite English word lol) of benefits to owning one:

  • A peace of mind
  • An organized life
  • All your information is stored in a single place
  • It will make a hard time easier for your family
  • It’s a place to document your final wishes
  • You can leave letters to your loved ones

What is in my end of life planner?

I designed a thorough end of life planner that’s affordable and multi-faceted. It was meticulously crafted with YOUR needs in mind.

You will find a lot of planners that are 100+ pages, however, that’s a lot of printing – have you seen the prices of ink (at least in my country)? So, I narrowed it down to about 60 pages for your convenience.

My planner has many features but I want to share the contents with you before we get to the specs.

1. Personal Information

The first category holds ALL your personal information in only 4 pages. 

These include:

  • Personal details 
  • Employment information
  • Marital status and details
  • Child information – includes possible medical conditions, guardianship, and school information, etc
  • Pet details – include guardianship, dietary needs, temperament, feeding, and health details, etc
  • Emergency contacts

2. Medical Information

Summarize all your medical details in 7 pages.

These include:

  •  Preferred health professionals – primary physician, hospital, and specialists
  • Your personal medical history – conditions, surgeries, and allergies
  • Vaccination records – Childhood, adult, travel, and seasonal vaccinations as well as booster shots
  • Family medical history
  • Health insurance details
  • DNR order (if applicable)
  • Organ donation information (if applicable)
  • Documents locations – physical and digital copies

3. Financial + Insurance Information

This is the most extensive category resulting in a whopping 14 pages!

These include:

  • Bank accounts and online banking details
  • Investments
  • Retirement information
  • Credit cards
  • Debt/loans
  • Life insurance
  • Home insurance
  • Vehicle insurance
  • Income report
  • Expenses report
  • Subscriptions/memberships
  • Utilities
  • Taxes

4. Assets

Assets consist of 4 pages in my end of life planner.

These include:

  • Real estate 
  • Vehicles
  • Valuables
  • Warranties

5. Digital Footprint

These 4 pages keep your passwords in one place. There is also a space for notes so you can inform your family what to do with these accounts in the case of unforeseen circumstances.

These include:

  • Social media
  • Subscriptions
  • Email accounts
  • Online accounts

6. Relationships

3 Pages with the contact details of your relationships – you are more than welcome to print more pages if you run out of space.

These include:

  • Family
  • Friends
  • Professional relationships

7. The Document Finder

The document finder is a single page that keeps the location of all your physical and digital documents. There’s also a checkbox for you to mark if you attached copies of said documents to the end of life planner.

8. End Of Life Wishes

These 8 pages will inform your family of your last wishes. It’s also a place to leave letters to your loved ones.

These pages include:

  • Funeral details
  • Service arrangements – includes church name, preferred clergy, scripture, and music
  • Eulogy details 
  • Burial instructions
  • Cremation information
  • Pallbearers
  • Reception details
  • Obituary information
  • Funeral cover with beneficiaries
  • Where should my possessions go?
  • A final letter from me
  • A letter to…
  • Space for other special wishes

9. Notes

I included 3 note pages – a solid lined, dotted lined, and grid page.
